01 — Wellbeing
This solution is primarily for those who need minimal support and with families looking for peace of mind.
We want to ensure you are not isolated in your own home and can support you to maintain your physical and mental wellbeing by providing you with access to the community and links of companionship with people of similar interests.
Whether its assisting with weekly grocery shopping, escorting you to the salon, going on day trips to your favourite places, or supporting you with getting involve in local community initiatives and clubs - we are here for you.
02 — Reablement
Our Wellbeing and Reablement package is there to support you on the transition following a hospital stay.
We offer a 6-week package for the period when you may need a little helping hand in returning home and getting back on your feet.
We can even start before you get home and prepare your home for your arrival - for example putting the heating on/clearing out the fridge/changing your bedding and stocking up with the essentials.
Once home, we can continue to provide additional support over a short period to help you return to ‘normal life’.
We can support you with meal preparation, personal care, grocery shopping and even with transport to any follow up hospital appointments. We also offer reablement through rehabilitation if this is required.
03 — Live well
We want to ensure you continue living in the comfort of your own home for as long as possible, and our Support Workers can help you to remain as independent as possible in this time.
Our trained staff can support you with your daily routines, personal care tasks and with light domestic activities around the home to ensure you can focus on what is important to you. We understand how important this is to maintain your emotional and mental wellbeing, and we can be there to ensure this is possible.
We can help you to engage in day to day activities, maintain companionship and social interaction with your friends and family, we’ll get to know you and find out about your hobbies and interests. Our Support Workers will also assist with meal preparation and provide ongoing access to the community to ensure you maintain the links with what is important to you.
04 — Reminiscence
We offer a four-part solution which covers the entire dementia pathway.
Early support and intervention for people with dementia can help individuals to retain as much independence and live fulfilled lives for as long as possible.
Our Support Workers can provide the support that can help you to remain safe at home for as long as possible. We provide the support needed to encourage your independence and maintain daily routines, keep you engaged within the local community, provide support to ensure nutritional needs are met and with the safe management of prescriptions.
We will help to maintain your care needs at home in avoiding unnecessary hospital admissions or long-term care admissions. The support we provide can progress as needed to meet your changing needs and our Support Workers will always uphold your dignity and respect whilst maintaining your comfort.
Our care team can also assist with future care planning, end of life planning, act as a healthcare liaison service and provide family support and dementia training.
05 — Complex Care
We are proud to provide industry-leading, specialist and high-quality personalised homecare for people with complex neurological conditions and respiratory conditions. We provide a person-centred package delivered by our highly trained and skilled carers.
All of our complex home care packages are supervised by nurses, with advice from consultants and clinical experts who collectively have decades of experience in leading neurodisability services.
